Admissions Intake Process
Welcome Home! Recovery is a lifetime journey. From inpatient to outpatient care, sober living housing or an alumni support program, our mission is to walk that path with you.
Your journey begins with an initial confidential phone conversation to verify insurance and tentatively schedule your admission followed by a discussion of your current substance abuse and mental health concerns. Our medical and clinical teams review all assessments to confirm that our program is well-suited to your needs.
Once the admission assessment is complete, we will schedule your into our program and our staff will look forward to welcoming you into the Evolve family and ensuring a smooth transition into our care.
Evolve Residential
Our residential programs are fully staffed with experienced substance abuse
clinicians and recovery technicians. We offer a medium intensity residential level of
care which is typically 30-60 days as well as low intensity residential level of care
which is approximately 5 months in duration. Clients are able to begin recovering
from substance abuse while simultaneously building a strong foundation in
recovery. Our treatment approach is individualized to each client and clients are
encouraged to set realistic goals with help from staff to achieve them during their
time at Evolve Residential.
Intensive Outpatient Program IOP
An intensive outpatient program (IOP) is a structured treatment option for
individuals needing significant support for mental health or substance abuse issues,
offering therapy sessions multiple times per week while allowing participants to live
at home, on site, or attend IOP virtually. It provides structure and accountability to
build a life of long-term sobriety.
Outpatient Program OP
An outpatient program is a flexible treatment option where individuals attend
therapy sessions or medical appointments without residing in a facility, allowing
them to maintain their daily routines. It’s suitable for those requiring less intensive
support than inpatient or intensive outpatient programs and providing counseling
and support.
Recovery Housing
Recovery housing provides a supportive living environment for individuals in
recovery from substance abuse, emphasizing sobriety and peer support. Residents
typically follow house rules, attend meetings, and engage in community activities to
foster a stable and substance-free lifestyle during their recovery journey.
Mental Health Services
Outpatient mental health services encompass a range of therapies and treatments
tailored to individuals managing mental health challenges. These services often include counseling, medication management, and psychotherapy sessions scheduled at regular intervals to support clients in maintaining their mental well-being while continuing with their daily lives.
PHP
PHP stands for Partial Hospitalization Program, a structured form of outpatient
treatment that provides intensive therapeutic services for individuals who require more support than traditional outpatient care but do not need 24-hour supervision. Participants attend therapy sessions seven days a week and various fun activities on
the weekend for 28 days.
Therapies - Individual, group and Family
Mental health therapy involves sessions with a trained therapist to address
psychological challenges, improve coping skills, and enhance overall well-being.
Therapists use various approaches such as cognitive-behavioral therapy (CBT),
psychodynamic therapy, or mindfulness techniques tailored to individual needs to
promote healing and emotional growth.
